Social Prescribing Link Workers
Our team of Social Prescribers work with patients to identify what matters to them and to create a joint action plan to help them achieve their goals. They can give you the time and confidence to work on underlying issues that affect your health and wellbeing. They can also connect you to organisations and sources of support in the community and refer you to other organisations where appropriate.
Health Visitors
A health visitor is a registered nurse who has received training particularly related to babies, children and pregnant women. Their role is to provide families with children under five years old with support and advice around the general aspects of mental, physical and social wellbeing.
