If you’ve been off sick for more than 7 days:
If you have have been off sick for more than seven consecutive days, you may need to provide medical evidence to your employer.
If you are a registered patient of Quedgeley Medical Centre, you can request a sick note via the button below. Please note that this form will ask you:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
If you’ve been off sick for 7 days or less:
If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill.
You may be asked to fill out a form when you return to work; we call this self-certification.
Fit to return to work:
You do not need to see a healthcare professional again to go back to work. Your employer may ask for confirmation that you’re fit for work, but healthcare professionals do not have to provide this.
Talk to your employer if you feel well enough to return to work before the end date on your fit note, for example if you’ve recovered from your illness or injury sooner than expected.
