Viewing your GP record

You can view your GP record using online services (NHS App, AirMid app, SystmOnline).

If you do not yet have online access or you are not able to see your full record online, you can submit a form via the below button to request access or additional access.

Medical reports

The GP is often asked to write medical reports for:

  • Driver and Vehicle Licensing Agency (DVLA)
  • Home Office
  • Department for Work and Pensions (DWP)
  • Crown Prosecution Service (CPS)
  • Police
  • Solicitors / lawyers
  • Insurance companies
  • Local councils
  • Employers
  • Schools / education institutions

In most cases, we would expect the request for the report to come directly from the organisation who requires it. We will then respond with the specific information that is requested.

Please only submit a request for a report if you have been asked to do so.

If you are registered with Quedgeley Medical Centre, you can submit a form via the below button to request a medical report. Please note that this form may ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of
