Your Record

This page lists the information, guidance and resources available to help you access and manage your health information, or the health information of someone you care for.

Updating your information

It is important that your medical record is up-to-date with any changes to personal information, including title, name, address, phone number and email address.

You can update this information via your online record either through your NHS account or your SystmOnline account.

Alternatively, you can complete our online Change of Personal Details form.

Please note:

  • Changes of name may require proof of name change (e.g., marriage certificate). In such circumstances, we will contact you to request this.
  • If you move home, you will need to check that your new address still falls within our Practice boundary. If your address falls outside the boundary, we ask that you register with a GP practice closer to home.

Accessing your own record

In accordance with the General Data Protection Regulation 2018 and Access to Health Records Act, patients may request to see their medical records.

No information will be released without the patient consent unless we are legally obliged to do so.

Sharing your information with someone else

We understand that there may be occasions when you want a relative, friend or carer to have access to your medical information or permission to speak or act on your behalf.

This might be to help you manage your appointments, request medication, or understand what is happening with your care.

We have strict procedures in place for this, which ensure your confidentiality and dignity are protected.
